IntAc
Intensive Case Management Access Coordination
A centralized process and single point of access to Mental Health Intensive Case Management Services provided by four agencies for residents of Hamilton to persons, aged 16 years or older, who are living with a serious and persistent mental illness.
The four mental health agencies are:
- Canadian Mental Health Association (CMHA)
- Community Mental Health Promotion Program (CMHPP) – City of Hamilton
- Hamilton Mental Health Outreach (HMHO)
- Hamilton Program for Schizophrenia (HPS)
In order to be eligible for IntAc, the person referred needs to be:
- Living in Hamilton
- Diagnosed with a serious and persistent mental illness (Axis 1)
- Is under the care of a physician (some services require a Hamilton physician)
- Willing and able to participate by working on goals, and requires support to maintain wellness in the community
Who can refer to IntAc? Anyone! – There is no wrong door.
- Healthcare professionals
- Community / social service agencies
- Families / friends
- Self-referral
Steps in the IntAc Process
- Clients can self-refer, or professionals can refer by filling out common referral form
- The IntAc Coordinator will contact the client and meeting with him or her to obtain consents and gather further information.
- The IntAc Admissions Team meets every 3 weeks to discuss the eligibility of the client and which agency would be best suited for their needs and/or other resources that can be recommended.
- The referral source and client are notified of the outcome.
